Application for residency.

Complete the full application below and submit it online — no printing required. Please answer every question; if a question does not apply, enter N/A. The head of household completes this application; each additional adult (18+) will complete their own.

A few things to know before you start. There is a $25.00 non-refundable application fee for each adult (18+), paid by money order or certified check when your application is processed. For everyone's security, please do not enter Social Security numbers, driver's-license numbers, or bank account numbers here — bring copies of Social Security cards, birth certificates, photo ID, and proof of income and assets for all household members when our team follows up. We run credit, criminal, and sex-offender background checks to determine eligibility. This is affordable housing under the Low-Income Housing Tax Credit (LIHTC) Program.

Household composition

List every person who will live in the home with you, starting with yourself (Head of Household). Leave extra rows blank.

Please answer:

1) Does anyone live with you now who is not listed above?
2) Do you anticipate any additions to your household in the next 12 months?
3) Have you or any member of your household ever used a different name from those above?
4) Will any household member live in the rental unit less than full time?
5) Will any household member require a reasonable modification to the rental unit?

Household income

You must disclose all sources of income for everyone in your household. Leave extra rows blank.

6) Do you receive cash benefits from the Department of Human Services (DHS)? If yes, list your worker's name and phone number.
7) Are you employed? If yes, list your supervisor or HR manager's name, phone number, and your hire date.
8) Does anyone outside your household give you money? If yes, how much per month?
9) Does anyone outside your household pay bills or buy supplies for your household? If yes, estimated monthly value?

Assets

Disclose all assets for everyone in your household — checking, savings, CDs, 401(k), IRA, life insurance, real estate, stocks, bonds, etc. Do not enter account numbers here.

10) Have you or any household member disposed of an asset for less than fair-market value in the past two years?
11) Do you now, or have you or any household member ever, owned property such as a house, mobile home, or cottage? If yes, give the address and date purchased/sold.

Rental history

Are you or any household member related to your current landlord? If yes, who and what relationship?
12) Is your rent currently subsidized? If yes, do you have a Housing Choice Voucher, and which housing commission issued it?

Previous residence

13) Has any rental housing you lived in been destroyed or damaged by fire?

Supplemental information

Answer every question for all household members. If yes, please explain in the box.

14) Have you or any household member ever been evicted from a rental unit? If yes, list the apartment/owner name, phone number, and when.
15) Have you or any household member ever been taken to court or had a judgment against you for non-payment of rent or lease violations? If yes, explain.
16) Is any household member claiming a disability? If yes, explain any requested accommodation.
17) Is any household member temporarily absent (school, military, etc.)? If yes, who and anticipated return date?
18) Are you or anyone in your household currently, or ever been, on parole or probation? If yes, who?
19) Have you or anyone in your household ever been cited, arrested, charged, or convicted of ANY crime (misdemeanor or felony) other than traffic violations? If yes, list in detail regardless of date.
20) Are you or anyone in your household subject to registration as a sex offender? If yes, list name(s) and the address where currently registered.
21) Have you or anyone in your household ever used any name(s) other than your current legal name? If yes, list the name(s).
22) Have you or any household member ever been arrested for or convicted of any drug-related criminal activity (use, possession, distribution, trafficking, or manufacturing)? If yes, who and the circumstances.

I understand that the information on this application is collected to determine my household's eligibility for the LIHTC Program, and I authorize Elite Property Management, LLC to verify all information provided, including income, assets, rental history, credit, and criminal history. Acceptance of this application does not guarantee a rental unit; eligible households are placed on the waiting list. I will notify Elite immediately if my address, phone, income, or household composition changes.
